A story that begins with a phone call — and brings a person back to life

Volodymyr Tymofiiovych is 87 years old. He lives alone in the settlement of Pechenihy.
Due to the war, his relatives moved abroad, so he was left without daily support. Throughout his life, he worked as a driver — he was used to movement, the road, and an active lifestyle.
A few years ago, his health significantly worsened: walking became difficult, and over time he almost stopped leaving his home.
Volodymyr Tymofiiovych is registered with the Center for Social Services (CSS). Although he receives assistance from a social worker, it is becoming increasingly difficult for him to handle even simple everyday matters, including going to the store on his own.
Over time, he began to feel more and more loneliness and social isolation.
On the advice of his neighbors, the man contacted a case manager from the charitable organization “Charitable Foundation ‘Source of Revival’”, who works within the project “Basic Social Support and Strengthening the Capacity of CSSCs in Communities”.
Volodymyr Tymofiiovych did not ask for direct help — he only wanted to know where to apply and what documents were needed to receive a wheeled walker.
Because of his small pension, he could not afford to buy it himself.
The case manager took this case into work and found an opportunity to provide Volodymyr Tymofiiovych with the mobility aid he needed — a wheeled walker with a basket.
The result was immediate. The man started using the walker right away at home and in his yard, and later he was able to walk independently to the nearest store.
On the way, he meets neighbors, greets them, and talks with them.
Most importantly, in his own words, he no longer feels lonely or dependent on outside help in everyday life.
Stories like this show that case management is about timely support that helps a person regain independence and return to a full life.
